Most probably because you application is crashing.

the same happen whether i'm doing any project from the internet or using the examples (demos) for QT Creator..
but if my friends run the same program she succeed..
maybe my QT/MySQL have some prob.. or maybe just the pc ??

Or may be something like a broken Qt installation, have you tried reinstall Qt? (uninstall first) download link to open source Qt SDK for Windows
What happens if you run the .exe from windows explorer?
